TX - 2026 - Rural Texas Strong – Fact Sheet
Guidance / reference (not standalone opportunity)
2,895,576 rural residents
The Centers for Medicare & Medicaid Services approved Texas' Rural Health Transformation Program application and awarded Budget Period 1 funding as of April 7, 2026. The state is actively preparing procurements but has not yet issued public Requests for Applications or other procurements to local providers, maintaining fairness ahead of future opportunities. State opportunities: 4 active (1 open, 3 closing soon, 6 past/closed), $820.9M listed funding. Key opportunities: Rural Texas Strong Program – Initiative 4: The Next Generation of the Small Town Doctor and Team; The Rural Texas Strong Program – Initiative 6 (Part 1): Infrastructure and Capital Investments for Rural Texas; Family Support Services (FSS) Community Services Grant. Strategy alignment: supports workforce, prevention, care access priorities.
Texas is implementing the 'Rural Texas Strong' strategy under RHTP, led by HHSC and funded by annual federal awards (~$281M/year FY2026–FY2030). The approach is built on six coordinated initiatives: chronic disease prevention, patient-centered technology, advanced telehealth and AI, rural workforce investment, infrastructure and cybersecurity upgrades, and capital support for rural facilities. Competitive grants, procurement rigor, and ongoing oversight anchor implementation. Milestones align with CMS approval, application deadlines, and annual funding cycles.
Model
Multi-pronged, state-administered model leveraging Texas HHSC leadership to deliver six strategic initiatives—technology, patient engagement, AI/telehealth, workforce, unified infrastructure and cybersecurity, capital investments—distributed via Rural Texas Strong and federal RHTP cycles.
Key Initiatives
Timeline: CMS Notice of Award (December 29, 2025); Texas application submitted (November 3, 2025); Budget Period 1 approval (April 7, 2026); Annual funding FY2026–FY2030.
Rural Hospital Sustainability: Several programs target hospital debt reduction, infrastructure improvements, and workforce stability to maintain rural health access.
Maternal and Pediatric Health: Specific awards address maternal health outcomes and pediatric tele-connectivity to improve service quality for rural families.
Telehealth Expansion: Programs focus on telemedicine resources to connect rural communities with specialized care.
