the Rural Health Transformation Program (RHTP) “Connected to Care, Closer to Home” initiative. The program aims to expand health care access, strengthen care coordination, and improve rural health outcomes by integrating EMS personnel into community-based, patient-centered care delivery models. Up to three regional EMS Council service areas will be selected for initial implementation, with individual subawards up to $400,000—competitive selection based on application merit, not pre-selected regions. Approximately $3.7 million in federal funds are available for the initial budget period, with continuation funding possible through September 2030, pending performance and available funds. Eligible applicants include licensed EMS agencies, nonprofit organizations, state, local, or Tribal government entities, hospitals/health systems, FQHCs, rural health clinics, institutions of higher education, and other organizations with demonstrated capacity to implement CP/MIH in FORHP-designated rural Virginia areas. Applicants must have or be obtaining active SAM.gov registration and UEI. Awards require demonstration of local partnerships and operational readiness; minimum fundable application score is 70/100. Eligible applicants are licensed Emergency Medical Services (EMS) agencies, nonprofit organizations, state, local, or Tribal government entities, hospitals/health systems, Federally Qualified Health Centers (FQHCs), Rural Health Clinics (RHCs), institutions of higher education, and other organizations with demonstrated ability to implement Community Paramedicine/MIH programs. Applicants must have or be in the process of obtaining active SAM.gov registration (UEI required) and must serve communities in one or more FORHP-designated rural Virginia localities within a Regional EMS Council service area (per Appendix A of the RFA). Regional EMS Council areas for initial implementation will be selected competitively, not pre-specified. Minimum application score for funding: 70/100.
