Mississippi’s BRIDGE Innovative Pilot Program, funded under the Rural Health Transformation Program (RHTP), is a $16 million initiative managed by the Mississippi State Department of Health (MSDH) to strengthen rural healthcare systems statewide. It aims to support transformative, collaborative, and scalable projects that expand access to care, address social determinants of health, and improve outcomes for rural Mississippians. Eligible applicants include rural hospitals, Critical Access Hospitals (CAHs), clinics (RHCs, FQHCs, community health centers), behavioral health providers, EMS agencies, nonprofit community organizations, community colleges, universities, organizations serving families and vulnerable populations, and those addressing community-level health needs. Projects may include collaborative networks, school-based health, maternal and child health partnerships, behavioral health initiatives, nutrition-focused approaches, employer-partnerships, and innovative access models. Each entity may submit one application, capped at $2 million, with all project activities to be completed by July 31, 2027. Applications must be submitted online with required documentation; funds are reimbursed post-award except in certain advance cases. Review considers readiness, impact, statewide distribution, and alignment with measurable objectives and outcomes. Awards are anticipated in January 2027. All funded organizations must comply with federal and state regulations including 2 CFR 200 & 300 and support statewide health information exchange.
Eligible applicants are Mississippi-based entities or those currently serving rural Mississippi residents, including: rural hospitals and Critical Access Hospitals (CAHs); post-acute providers (nursing, home health, LTAC, rehabilitation, hospice); outpatient clinics (Rural Health Clinics, Federally Qualified Health Centers, community health centers); behavioral health providers; emergency medical services (EMS) agencies and emergency response partners; nonprofit community-based organizations focusing on healthcare access, delivery, public health, or care coordination; community colleges, colleges, and universities; organizations serving children, families, or vulnerable populations; specialty service providers and those engaged in care coordination; and other entities addressing community-level rural health outcomes. Applicants do not need to be physically located in Mississippi but must demonstrate current service to rural Mississippi clients/patients and project impact on rural patients.
