Program Overview
Iowa’s Rural Health Transformation Program, known as Healthy Hometowns, is currently implementing a multi-year plan focused on establishing a sustainable rural health system through federal CMS funding. Five core initiatives—Hometown Connections, Combat Cancer, Communities of Care, Health Information Exchange, and EMS Community Care Mobile—target improved access, care, and outcomes for rural populations. Funds will flow through states, which must submit a Rural Health Transformation Plan to CMS by Dec. 31, 2025, to access and manage these resources. The bold and comprehensive proposal is aimed at building a high-quality, sustainable system of care that improves the health, well-being, and quality of life in rural communities across the state. If fully awarded, Iowa could receive $200 million per project year period (a total of $1 billion over the 5-year grant). Healthy Hometowns Five Key Initiatives List items for Healthy Hometowns 5 Key Initiatives Hometown Connections An initiative that builds formal partnerships to restructure healthcare delivery options for rural communities. This includes an expansion of Iowa Governor Kim Reynolds' Centers of Excellence Program and a rare opportunity to develop enhanced Health Hubs, or hub-and-spoke networks of care, with investments in telehealth, specialized medical equipment, provider recruitment and retention, efficient space utilization, and limited funds to support care for uninsured Iowans. Health Hubs may include school-based service provision. This initiative also includes Best and Brightest, a sub-initiative to recruit and retain an excellent rural healthcare workforce. For questions, contact HealthHubs-TA@uiowa.edu Combat Cancer: Prevent and Treat An initiative to comprehensively tackle cancer throughout the state via increasing access to cancer screening, forming cancer-specific Health Hubs, upgrading equipment for cancer screening and treatment, delivering supportive care for families impacted by cancer, and supporting studies and analyses by academic partners. Iowa will address lung cancer prevention through radon testing and mitigation, breast cancer by paying for mammograms and follow-up breast MRIs, colorectal cancer through FIT tests and follow-up colonoscopies, skin cancer through telehealth and new equipment, and prostate cancer through routine screening methods. Communities of Care An initiative that supports co-location of different rural provider types for convenient patient access and improved coordination, hires community health workers as system navigators, and invests heavily in chronic disease prevention and management techniques. Health Information Exchange An initiative that allows records to be accessible across the state as patients travel throughout new Health Hubs and seek care in new ways. EMS Community Care Mobile An initiative that invests in new telehealth technology for high-risk transport of mothers and their new babies to higher levels of care throughout the state and a mobile integrated healthcare program that brings prenatal, postpartum, post-surgery discharge, chronic disease management, and other types of care to rural residents in their homes or to easily accessible sites in their communities. The Agency encourages Respondents with experience in particular segments of the services or products described herein to respond to Agency questions with information related to best practices, industry standards, technology issues, qualifications and capabilities of potential vendors, and any other information the respondent deems responsive. Respondents are asked to complete the attachment titled Submissions in Response to RFI# RFI# COMPADM26007 and submit it to the Agency’s Issuing Officer by the due date. This RFI does not commit the Agency to publish a procurement, such as a request for proposal, or award any contract. This RFI is not a pre-qualification process for any future contract solicitation. 1.2 Background Iowa’s rural communities face persistent and intensifying public health challenges, including limited access to care, workforce shortages, and disparities in health outcomes. In response, the Agency developed the Healthy Hometowns initiative 1 as the state’s submission to the federal Rural Health Transformation Program (RHTP), authorized under the One Big Beautiful Bill Act. 2 This initiative is designed to transform the delivery of healthcare in rural Iowa by building a high-quality, sustainable system of care that improves health, well-being, and quality of life for rural residents. Through the Iowa HHS Strategic Plan and Strategic Plan in Action 3 , the Agency has committed to elevate organizational health, advance operational excellence, and help Iowa thrive. The Iowa Rural Health Transformation Plan, Healthy Hometowns, is in direct alignment with those goals by promoting access to health and human services resources and helping individuals, families, children, and communities thrive. Iowa Governor Kim Reynolds set the foundation for Healthy Hometowns via House File 972 of the 91st Iowa General Assembly. The vision for Healthy Hometowns is to implement a robust Hub and Spoke framework that supports long-term, sustainable and high-quality health care for Iowans living in rural areas. Healthy Hometowns has three primary goals: 1. Iowans will be able to get health care within their rural communities at the most appropriate locations for type and level of care thanks to support from newly developed partnerships, more rural primary care physicians and specialists, and upgraded equipment. 2. Iowans living in rural areas will have improved health outcomes with similar rates of morbidity and premature mortality to those living in Iowa’s more populous areas. 3. Iowa will invest in the development and utilization of innovative technology and data infrastructures to support sustainable care options close to home, seamless care partnerships, and data sharing throughout the state. The purpose of developing Hub and Spoke networks of care for cardiovascular healthcare is to increase access to and availability of cardiovascular care in Rural Iowans. Through the Healthy Hometowns Rural Health Transformation Program, Iowa is working toward innovative and transformative models of healthcare delivery to best serve rural Iowans. Hub and Spoke networks typically work collaboratively to increase access to cardiovascular healthcare, including associated preventive care, in Rural areas in a manner that keeps care available locally when appropriate, while simultaneously increasing patient volume and long- term sustainability of rural healthcare providers. 1.3 Definitions. Event schedule: - Notice of Intent to Release To all interested parties: The Iowa Department of Health and Human Services (the Agency) is providing this notice of intent to release a request for proposal (RFP) on or around May 8, 2026; to procure RFP# COMPADM26008 Combat Cancer Health Hub Program, Round 2. This procurement is for establishing the framework for hub and spoke networks of care for the delivery of cancer care in Iowa. The Agency anticipates that this work may include forming rural provider strategic partnerships, advancing telehealth, establishing referral networks and agreements (particularly among non-affiliated provider organizations), provider recruitment, equipment purchases and associated minor renovations, and other funds needed to establish or advance the provision of cancer care at spoke sites providing routine cancer care and hub sites providing specialty care and advanced cancer care.
