the Rural Health Transformation Program to support Rural Community Health Integration (RCHI) — Initiative 1 of NY's five-year RHTP. RCHI establishes coordinated rural health partnerships to improve care coordination, patient access, financial sustainability, and health outcomes across rural communities. Applications may request up to $500,000 per county for planning and up to $3,000,000 for up to three implementation projects. The minimum geographic unit is a county; multi-county collaborative applications are encouraged. Funded by CMS/HHS as part of a $212,058,207.80 total federal award. Implementation update: Ellenville Regional Hospital is using a $3,000,000 RCHI award to implement NY-VISTA (New York Virtual Integrated Specialty Telehealth Access), an 11-hospital rural specialty-access collaborative. The model combines shared clinical workflows, telehealth and EHR infrastructure, workforce training, and common performance measurement while preserving local clinical decision-making.
